ANNUAL PHYSICALS

Thank you for putting your trust in us. At Rutland Family Healthcare, your health and continued well-being are our primary concerns. We strive to offer you the highest quality of care, and we continue to evaluate ways to better serve you. For best comprehensive care, all patients with a chronic condition or on a daily prescription medication, regardless of age, should receive a physical exam on an annual basis.  Without doubt, consistent annual examinations obviate a myriad of life threatening conditions, while they also build rapport between the patient and doctor. At your physical, we are able to perform a comprehensive exam, administer or recommend appropriate screening tests, coordinate your specialists, and provide you with current best practice guidelines.  Please work with our office to arrange your annual physical if you have not done so in the past year.

MISSED APPOINTMENTS

Keeping your regularly scheduled appointment is important – for your health, for the convenience of other patients, and for more efficient operations at our office. If you must cancel or reschedule your appointment, please let us know 24 hours before your scheduled time. There is a $35.00 "No-Show Fee" that will be charged to your account if we are not notified of a needed cancellation. After three no-shows, we do reserve the right to terminate the practice-patient relationship.  Your health is our top priority, and if you are routinely unable to keep your appointments with our practice, we want you to find another practice that will be more able to meet your scheduling needs.

MEDICATION MONITORING

Our office has chosen to use MedComp Sciences and/or Labtrust to provide therapeutic monitoring laboratory services. This test will provide both you and your provider with information meant to improve your health and well-being. Testing is done through a routine urine sample, and monitors for therapeutic levels of many commonly prescribed medications, both controlled and non-controlled, as well as several illegal substances. We want to ensure that the medications we are prescribing are being taken as directed, and that no new medications could possibly interact with any other substances in your system.  

 

Patients will receive an explanation of benefits (EOB) from their insurance company regarding the charges for these tests, with the name of the laboratory as MedComp Sciences and/or Labtrust.   • ANNUAL SCREENING

       New Patients

       Annual Physicals (If not screened as a new patient within the past 3 months)

  • SIX MONTH SCREENING

       Patients on at least one routine Schedule III or IV medication. This includes, but is not limited to: Ambien, Xanax, Valium, Soma,                  Flexeril, Ativan, Tramadol
 

  • THREE MONTH SCREENING

        Patients on at least one routine Schedule II medication. This includes, but is not limited to, Norco, Adderall, Concerta, Oxycodone

REIMBURSEMENT FOR SERVICES RENDERED

Our office accepts numerous insurance plans. We also offer discounted rates for patients without current insurance coverage.  To qualify for the discounted rate, payment must be rendered in full at the time of service. If there is a copay associated with your plan, it will also be required at the time of service. Once insurance is filed, if there is any additional patient responsibility, you will receive a bill for same. It is our policy that after six monthly statements without payment or a payment plan being approved by our billing department, your account will be turned over to a collection agency. If your account is not in good standing and turned over to a collection agency, we will no longer be able to continue to provide our services to you.

 

BILLING

Our practice outsources our medical billing to Premiere Medical Billing & Consulting, LLC.  We work closely with their office to ensure timely filing of visits and procedures.
